Oak Avenue Home accommodates adults (18+ ) with learning disabilities and Mental Health needs. Oak Avenue Home is a five bedroom supported living with a spacious living room, kitchen, bathroom and garden. Our supported living services provide innovative person-centered support enabling individuals to live independently where possible. Everything we do at Oak Avenue Home is guided by the people who use the ser-vice. We offer flexible and responsive packages of care support. The level of support can range from a couple hours per week to 7 days a week with rent in line with Housing Benefits.

Developing Daily living Skills

At Oak Avenue Home we support all aspects of daily life to develop the skills required to live more independently. This includes

• Prompt /Supervision with Personal Care and eating healthy

• Taking medication, maintaining the home, paying bills and budgeting

• Maintaining tenancies and support with benefits

• Employment and further education and engagement in community

Activities and working towards their goals and aspiration.

Our Team

At Oak Avenue Home our staff have the skills, knowledge and the experience to enable service users to look after their health needs and support service users to take more control of their recovery and make a difference in their lives. We work in partnership with the local authority, commissioners, social care and health care teams. We are transparent and have developed good rapport with the local authority.

At Oak Avenue Home, we provide 24 hours service, 7 Days a week to ensure that all our service users are safe. We provide tools and techniques to help service users take control of their health and manage their conditions better on a daily basis. Some of the activities we offer are: Bowling/swimming/Cycling, gardening, recycling,

Fitness Exercise, cooking and Life skills programs.

Oak Avenue Home accepts planned and emergency referrals. These re-ferrals will be assessed by the service manager to determine the level of risk and needs of the individual. Our packages of support and enable-ment are flexible and tailored to individuals’ needs. We offer advice about accessing benefits, funding and working out budgets
